What to expect when you call our office for an appointment…………
By now you have decided that you would like to come in and see one of our providers.
We would like to make your transition into our office as smooth as possible. Your initial telephone call will take approximately five minutes, please be prepared to provide our new patient coordinator with the following information:
- Your name / address / contact information
- Employer name and phone number
- Insurance company / subscriber name (if you are not the insurance subscriber)
- Date of Birth / (subscribers Date of Birth)
- SSN or Insurance ID # / group # / customer service #
- Previous dentist name & number
- Allergies / medical alerts
- Dental concerns / last dental check up or visit
- Do you have a doctor preference
- Who referred you to us
We must verify your insurance and benefits prior to your appointment. If we are not able to verify your insurance prior to your appointment, we will ask for full payment from you at the time of treatment and the insurance will reimburse you.
